![]() The KAP is a national repeated cross-sectional survey of the Israeli adult population conducted over the phone. Data on tobacco were derived from the Israeli Knowledge Attitudes and Practices (KAP) survey, which utilizes a stratified random sample design. This study utilizes a repeated cross-sectional design to explore the association between cigarette prices and smoking prevalence and intensity. Of note, in terms of tax structure, is the change in the tax base for ad valorem from retail price to wholesale price in 2009. If the prices remain low regardless of the tax share, they are going to affect consumption positively. Though this is above the WHO recommendation of 75%, a more relevant measure is the absolute price. Currently in Israel, the taxes on cigarettes are ~80.1% retail price compared to 80.8%, on average, in the European Union (EU). Research on low, middle, and high income countries has found that increasing cigarette prices is a cost-effective measure to lower smoking prevalence, thereby lowering the detrimental health and economic effects of smoking. Numerous countries have increased taxes to raise the prices of tobacco products to reduce the prevalence of tobacco use. Furthermore, while national data from Israel indicate an overall declining prevalence of smoking (22.5% in 2016), it is still high, particularly compared to many other high-income countries. The WHO predicts that smoking prevalence in Israel will further decrease to 21.9% by 2025, which reflects an 18.6% relative reduction from the level in 2010 thereby not meeting the 30.0% benchmark suggested by the WHO. Thus, increasing cigarette prices will likely lead to a reduction in cigarette smoking thereby improving public health in Israel. The total price elasticity of cigarette demand, given by the sum of price elasticities of smoking prevalence and intensity, showed that a 10.0% increase in the price is associated with a 4.6–9.2% lower cigarette consumption among Israeli adults. Similar results were found with the Marlboro cigarette prices. Moreover, a one unit increase in the price of the local brand of cigarettes was related to consuming 1.49 (95% CI −1.97, −1.00) fewer weekly cigarettes, controlling for household income and covariates. Analysis revealed that a one-unit increase (Israeli currency) in the price of local brand of cigarettes was related to 2.0% (OR = 0.98 95%CI 0.98, 0.99) lower odds of being a current smoker, adjusting for covariates including household income. The second step examining smoking intensity as the dependent variable, used OLS regression. The first step of the model utilized logistic regression with current smoking as the dependent variable. Multivariable analysis was employed using a two-part model while adjusting for covariates. The dependent variables were current smoking (yes/no) and smoking intensity, defined as the number of cigarettes smoked per week. These two price variables were the primary independent variables, and we adjusted for inflation. Once reunited, children may seem to be recovering. ![]() ![]() We also need to help parents appreciate that adults caring for these children need to have trauma-sensitive responses in order to help alleviate some of the extreme stress these children have experienced. We need to appreciate just how traumatized children are when they experience forced separation from their parents as is happening in this immigration crisis. It was only after he told them to measure the children’s resting heart rates, an indication of their internal stress, that they were convinced that the children were indeed under extreme internal stress (the heart rates were in the 160s instead of being in the normal range of the low 100s). They thought that behavior was indicative of them being just fine. Perry for being so concerned when the children looked outwardly relaxed. After the initial stressful responses when separated from their parents, the children appeared to be calmer and able to play and interact with the adults around them. If anything, their brain has been so overwhelmed they go into an unnatural state of “contrived calmness.” Early abandonment experiences are one of the most significant types of trauma a child can experience.Ĭhildren need and deserve to be securely attached to their primary caregivers, experiencing the cocooning aspect of a safe, predictable, loving relationship. Sometimes adults misinterpret children’s calmness after the initial terrified responses as an indicator that they are adjusting. They also may appear to be able to recover enough to function, but this is a protective reaction that allows them to engage in order to survive. Their dissociation happens as they are unable to cope with the depth of their fear from abandonment and the resulting emotional pain. The brain shutting down occurs because their pain is so great that after the initial crying, screaming, pleading responses, children often “disappear” into a state of dissociation. In many ways the mind and the brain have to shut down. ![]() What happens in the child’s brain is complex. Abandonment causes a kind of instant paralysis that leaves a child in a state of utter despair. When an infant, toddler or young child experiences abandonment, as happens when children are separated from their parents, they are overwhelmed by their sense of terror. Their wounds may not heal but instead float beneath the surface of their lives right into adult hood. When children experience feelings of disconnection, they do not have the defenses to fall back on what we as adults do. Any threat or disruption to that relationship arouses this primal fear, a fear that is embedded in the hardware of our brains, a fear we carry into adulthood. For the infant, maintaining attachment to its primary caretaker is necessary for its survival. Abandonment is a fear that we will be left alone forever with no one to protect us, to see to our most urgent needs. As infants we lay screaming in our cribs, terrified that when our mothers left the room, they were never coming back. It is a primal fear-a fear universal to the human experience. I think the answer lies in some of the research around abandonment.Īccording to therapist Susan Anderson in her book, The Journey from Abandonment to Healing:Ībandonment is our first fear. What is it about separating children from their parents that causes such a visceral response in each of us? It has been a call to action, and hopefully, the actions will both bring children and parents who were separated back together and prevent this cruel practice from happening ever again. The outcry seems to have awakened the deep moral code that drives us to protect and safeguard vulnerable children.
